Job summary
Join our dedicated Children's Diabetes Team at Somerset NHS Foundation Trust, where you'll play a vital role in supporting children and young people with diabetes and their families. The team provides a cross-county service, and you will be based on the Children's Unit at Musgrove Park Hospital, delivering specialist advice, education, and care in hospital, community, and school settings.
This is an exciting opportunity for a registered children's nurse with experience in paediatrics and a passion for supporting children with diabetes. There is an option for this post to commence as a Band 5 development post, depending on your previous skills and experience.
You'll manage a defined caseload, support families in hospital, home, and school settings, and work closely with a multidisciplinary team. The role involves empowering families in diabetes management, supporting transition to adult services, and contributing to service development.
Travel is required, and you may also be asked to support services across the county. A car user allowance is provided.
Main duties of the job
- Deliver specialist nursing care, education, and support to children and young people with diabetes and their families.
- Manage a defined caseload, providing care in hospital, home, and school settings.
- Work collaboratively with a multidisciplinary team including paediatricians, dietitians, psychologists, youth workers, and administrators.
- Support transition planning from paediatric to adult diabetes services.
- Provide training and advice to hospital and community staff on diabetes management.
- Respond to urgent queries and provide clinical advice to prevent complications and hospital admissions.
- Maintain accurate clinical records and contribute to audits and service evaluations.
- Promote evidence-based practice and contribute to policy development.
