Senior Rehab Assistant/ Assistant Practitioner

Job summary
We are looking for a B4 Assistant Practitioner to work alongside the Physios and OTs in the Community Rehab Team in Taunton and West Somerset carrying out assessments for patients for rehab and creating rehab programmes for them to follow. You will be responsible for delegating plans to Rehab Assistants within the team.
The main focus of the Community Rehabilitation Service is to provide rehabilitation support to patients within their own communities.
We enable our patients to regain any lost skills, increasing their levels of independence and wellbeing, and empower and equip them to make lasting changes for their future.
We will give them support and guidance to help them to achieve their goals by encouraging them to contribute to decisions about their treatment.
