We are seeking a dedicated Mental Health Practitioner to join our forward-thinking Community Mental Health Service within Somerset NHS Foundation Trust. In this vital role, you will provide high-quality, evidence-based psychosocial interventions to individuals with severe and enduring mental health challenges. • Establish and maintain effective, therapeutic relationships with service users, carers and families.

  • Work collaboratively with a wide range of partners including GPs, social care, voluntary organisations and other mental health services.

  • Contribute to the clinical leadership of the team, offering supervision and guidance to junior staff as agreed with the Team Manager.

  • Identify and respond to safeguarding concerns and manage clinical risk in line with Trust protocols.

  • Create and communicate personalised recovery and risk management plans.

  • Participate in team meetings, service development initiatives, and clinical governance activities.

  • Maintain accurate clinical records using electronic systems (RiO).
