Our service covers the urban and rural areas of West Somerset, from the Quantock Hills to Exmoor. These positions are dayshifts and are usually 8am-4pm, 12pm-8pm or 8am-9pm. We offer flexible working solutions where possible to ensure a work-life balance and part-time hours will be considered.You'll need to be a car driver with your own vehicle to visit patients. Main duties of the job
As a community RMN, you'll provide care to patients in their homes, managing a range of complex health issues and collaborating with various other disciplines. Triage referrals, completing initial assessment to understand what is needed and plan what the involvement from IDS will be. Referrals on to other services as appropriateYou'll also work closely with the OPCMHT, D2A and other services, focused on preventing acute admission. You'll develop a broad range of nursing skills, in support the management of behaviours that cause concerns.You'll need to make autonomous clinical decisions and create holistic care plans, with the support of your colleagues and Team manager
