Band 6 Occupational Therapist Community Rehab Service. Taunton and West Somerset.
May suit B5 looking for developmental post into B6 role.
The Community Rehabilitation Service focus is to provide rehabilitation support to patients within their own communities to achieve what is important to them.
We enable our patients to regain lost skills, increase their levels of independence and wellbeing and to empower and equip them to make lasting changes for their future.
We give them support, guidance and tools to help them to achieve their goals and encourage them to contribute to decisions about their treatment.
We are a multidisciplinary team consisting of Occupational Therapists, Physiotherapists, B4 and B3 Rehab support staff and a Pharmacy Tech and work closely alongside other community and voluntary sector services.
Main duties of the job
Job Purpose:
To provide high quality patient-centered care and specialist Occupational Therapy rehabilitation to adults referred to the Rehabilitation Service.
To have a patient centred approach and provide specialist assessments and treatment planning to enable patients to choose and work towards meaningful goals following creating a problem list and treatment plan.
Participate in the development of the Rehabilitation Service in the community and within the Community hospital bedded environments.
To provide specialist advice regarding the rehabilitation management of patients to families, carers and other members of the MDT.
To delegate rehab delivery, as appropriate, to the Rehab support staff.
Provide support, guidance and training to Band 5 Therapists, Apprentices, Rehabilitation Assistants and Students.
Responsibility for a caseload of complex patients within both the community and the community hospital bedded environments.
