We have exciting opportunities for Registered Nurse's to join our team who have a passion for providing urgent care in the patients' home. You will be a dynamic individual who is able to work with unpredictable caseloads to assist in avoiding Hospital Admission's wherever possible.
We are seeking proactive Registered Nurses looking for a new challenge to join our Rapid Response Team which provides an Urgent Community Response to both support and provide unplanned short term care interventions to patients in their home environment.
The Staff Nurse will be able to work effectively as part of a team and will always act within the limits of their competence, knowing who to ask for help and support as required. Staff Nurses will operate within agreed ways of working, following the relevant standards, policies and protocols used in their workplace including the standards in theNMC Code; Professional standards of practice and behaviour for nurses and midwives.
Main duties of the job
To provide and maintain a high standard of evidence based, patient centred care. To demonstrate good interpersonal skills and to develop practical skills required within the speciality, whist reflecting the philosophy of care.
To deliver patient care on a daily basis, using the systems in place in the department for a multi-disciplinary approach to patient-centred care delivery, which builds on the philosophy of greater involvement of patients and their relatives/carers by using the Trusts patient and public involvement strategy.
Experienced Band 5 post holders will also provide effective clinical and managerial leadership of the department, in the absence of the senior members of the team, which empowers staff to deliver patient- centred, evidence based nursing care
