Learning Disability Clinical Lead Nurse

Somerset NHS Foundation Trust

The closing date is 24 March 2025

Job summary

Are you an experienced qualified learning disability nurse (RNLD / RNMH / equivalent) looking to take a new step in your nursing career?

We have an excellent opportunity for an experienced, innovative and compassionate learning disability clinical lead nurse to join our team and provide expert leadership in delivering high quality, person-centred care, supporting the delivering of a new Acute Learning Disability Strategy across the two sites of Musgrove Park Hospital (MPH) and Yeovil District Hospital (YDH). You will be able to make a significant impact on delivery of exceptional care within service by managing a county-wide team of learning disability clinicians to support the acute settings in making reasonable adjustments for people with learning disabilities and supporting the people who matter to them, appropriate care planning and reducing health inequalities.

Main duties of the job

The role will include ensuring that the day to day management of the service is efficient and effective, reviewing and implementing new care pathways that will support safe and effective care and treatment for patients who have a learning disability, working with service groups to reduce inequalities in health and ensuring agreed care pathways are embedded across the organization for a seamless approach to the care of people with learning disability.

The role of this service is to reduce the barriers experienced by those with a learning disability in accessing acute healthcare, enhancing their experience and leading to improved health care outcomes by reducing inequality. You and the team will provide specialist advice to care teams on acute sites, supporting, empowering and signposting them in the care of adults with complex needs across MPH and YDH. This will include outpatients and Emergency Departments.

This is an expanding service that will require a passionate and authentic leader who is confident in driving change and innovation under the direction of the Operational Lead Nurse. You will be able to review and analyse data that will support service provision and the adherence to statutory and national requirements.
