Job summary

An exciting opportunity has arisen to join the Community Rehabilitation Service in Frome. This role is part of a wider community rehab service within Mendip.

Do you enjoy supporting patients to be independent and empowering them to understand their health conditions?

Do you enjoy participation and running of groups such as exercise classes?

Come and join our friendly Community Rehabilitation Team which consists of Physiotherapists, Occupational Therapists and Rehabilitation Assistants and Pharmacy Technicians. Together we can make a difference.

Main duties of the job

To work as an integral member of the Community Rehab Service in Mendip providing effective support to Occupational Therapists, Physiotherapists, and other team members

To independently assess, treat, review and discharge service users within the Community Rehabilitation Service

To work with patients undertaking therapy/rehabilitation programmes to support and encourage them to achieve their optimum level of independence and quality of life, as directed by team therapists

To work with patients in a range of community locations, including community hospitals, care homes and in service users own homes with support from rehabilitation professionals.
