Job summary

We are looking to appoint a Diabetes Support worker who is enthusiastic, motivated, with a passion for caring to join our dedicated Diabetes Team based at Yeovil District HospitalThis is a brand new role which provides support for the Adult Diabetes team and the adults with diabetes under their care.Full support will be given to the successful candidate to fulfil the role and integrate into our friendly and dynamic team.

The role focuses on technology in Diabetes and engaging with patients across the service. The role will involve working within both a clinical and administrative setting and the successful candidate will be required to travel to local community locations within Somerset.

We are looking for someone to work 30 hours over 4 days Monday to Friday.

As a health care Assistant you will work as part of a large multidisciplinary team and will play a key role in delivering the fundamentals of care for patients under the supervision of a nurse, whilst supporting patients to achieve the shared goals. Individuals should be able to demonstrate excellent communication skills who are willing to learn and take on new challenges as the service develops.

Main duties of the job

The purpose of this role is to provide support for the adult diabetes team and the adults under their care. The post holder will undertake a range of technology-based, clinical, administrative, and organisational duties to support the nursing staff and patient case load. Patient care is delivered across Somerset encompassing inpatient ward, clinic, and community hospital settings - a valid driving licence and access to own car is essential.

A key aspect of the role is setting up our patients with the technology needed to manage their diabetes and providing ongoing support to link with the diabetes team, supporting problem-solving to enable equitable access to technology. The post holder would be expected to be aware of the wider services available in communities and hospital and signpost patients as appropriate.

Administrative/organisational duties are a significant part of this post. The post holder will lead on various services and systems such as, but not exclusive to updating patient lists; liaising with technology companies to coordinate technology starts for patients; triaging phone calls and e-mails; providing ongoing technology support and advice to our patient cohort; creating resources to promote the support available to our patients.
