Job summary

We are a friendly and enthusiastic Older Persons Mental Health Team based in Sedgemoor, with two team bases in Bridgwater and Burnham on Sea.

We are looking to recruit a motivated Band 5 community mental health or allied health professional to join our multi-disciplinary team.

We are prioritising someone who has an energy and passion for working with older adults, and who would enjoy the opportunity to work alongside healthcare professionals in the wider neighbourhood team, including our District Nurse and CRS colleagues. This provides an excellent chance to work in an integrated way, improving client care and practice.

Main duties of the job

The post holder will be responsible for key working (under Supervision), a caseload of clients aged 65 years and above, with both functional and organic illness.

There is a Duty service provided by the team Mon-Fri 9-5pm, and the post holder will participate in our triage process, working carefully to ensure our clients are directed to the right support they need after referral to our team.

And there will be an exciting opportunity to undertake some Memory Assessments, and to become involved in our Liaison Service linking in with Weston General Hospital, Bridgwater Community Hospital and Burnham on Sea Community Hospital.

We are really keen to support our own clinicians with professional development, and so the successful post holder will also have the opportunity for further training and will be encouraged to participate in supporting students, and in supervising other staff.
