Job summary

To provide effective support to Occupational Therapists and Physiotherapists to deliver quality rehabilitation plans.

To work with patients undertaking rehabilitation programmes to support and encourage them to achieve their optimum level of independence and quality of life, as directed by team therapists

To work with patients in a range of community locations, including community hospitals, care home rehabilitation units and mainly in service users own homes with overall supervision from rehabilitation professionals.

Main duties of the job

You will be based in Bridgwater hospital community rehabilitation service. Your work will support Occupational Therapists and Physiotherapists to carry out specific rehabilitation interventions, working towards patients own personalised goals.

You will see patients in a range of community locations carrying out activities to promote independence, deliver exercise programmes and support personal and domestic activities of daily living. You may deliver equipment to patients and practice their use with it.

You will carry a caseload of patients and have supervision with a therapist to oversee your practice.

  • A good understanding of health and social care models.
  • An understanding of the physiology, psychology and organisation and function of the human body

Desirable

  • Working alongside Occupational Therapists and Physiotherapists
