Main duties of the job
Medicines Optimisation is all about supporting patients so that they get the best possible outcomes from their medicines. As a clinical pharmacy team we have a vision to ensure that medicines optimisation is at the heart of every patients experience and care.
You'll join our friendly team of assistants, technicians and pharmacists toto deliver ward-based Pharmacy Medicine optimisation services and dispensary support. This role will include Medicine reconciliation, assessment of patient's own drugs, counselling patients about their medicine, discharge facilitation and ward and dispensary-based medication supply.
