Join our dynamic and caring team at Yeovil District Hospital, part of the Somerset NHS Foundation Trust. We are committed to delivering high-quality patient care and rehabilitation services within our Acute Medical Unit (AMU). As a Rehabilitation Assistant, you will play a crucial role in helping patients regain independence and achieve personal rehabilitation goals.
Main duties of the job
In your new role, you'll assist patients with personal, domestic, and social tasks as part of their therapeutic plan. Working closely with Occupational Therapists, Physiotherapists, and other members of the multidisciplinary team, you will support patients in achieving their rehabilitation goals. Your key responsibilities include:
- Assisting patients in regaining skills for daily living activities.
- Implementing therapy plans set by Occupational Therapists and Physiotherapists.
- Providing feedback on patient progress and participating in team discussions.
- Using manual handling equipment to ensure safe patient care.
- Supporting the discharge planning process to enable patients to return home safely.
- Maintaining accurate patient records and attending regular training sessions.
