Job summary
Advanced clinical practice is delivered by experienced, registered health and care practitioners. It is a level of practice characterised by a high degree of autonomy and complex decision making. This is underpinned by a master's level award or equivalent that encompasses the four pillars of clinical practice, clinical leadership and management, education and research, with demonstration of core capabilities and area specific clinical competence. The expectation is that the postholder will spend 80% of their time involved in the clinical aspects of the role. An essential requirement of the role is working collaboratively with the ward sister/charge nurse
Main duties of the job
*Communicate very complex clinical information both written and verbal, in an easily understood form, to patients, carers, and all members of the multi-disciplinary team
*Impart / explain the prognosis / outcome of the patient's condition to them and, at times, their family in a sensitive and supportive manner
*Develop a close professional working relationship with the ward sister and all members of the multidisciplinary team, through respect for, and understanding of, their different roles and training. Working closely with stakeholders and clinicians to continue to improve care and service, in line with the NHS Plan.
*Working with Doctors, Nurses, Neighbourhood services and community stakeholders to ensure the needs of the patient and family are placed at the centre of care delivery.
*Work collaboratively to establish county wide community advanced practice agenda.
*Acting as a role model and highly competent practitioner whilst facilitating development of all staff within the team
*Promoting an innovative and progressive attitude to the continual improvement of patient care through research, evidence- based practice and teaching.
*Facilite and delivering education, training and clinical leadership for the advancement of clinical practice
*Promoting high quality evidenced based practice within the ward and the achievement of development, research and audit goals throughout the Directorate
*Establish links and networks with other professionals in similar roles locally and nationally.

Advanced clinical practice embodies the ability to manage clinical care in partnership with individuals, families and carers. It includes the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance people's experience and improve outcomes.
This definition therefore requires that health and care professionals working at the level of advanced clinical practice will exercise autonomy and decision making in a context of complexity, uncertainty and varying levels of risk, holding accountability for decisions made' (Health Education England, 2017)
To support the wider inpatient clinical team, by providing assessment and management of inpatients during admission, discharge and episodes of clinical concern. Ensure all care is person centred and length of stay is minimised. To be central in developing a new model of care for people that integrates community hospitals to people's homes.
As an experienced ACP, take the lead as the senior clinician on the ward and take responsibility for being the senior decision maker in patient care and management. This will be under the supervision and guidance of the doctor who may be supporting remotely from another site.
Demonstrate the ability to demonstrate advanced clinical practice, service development and improvement, initiating and disseminating research and education.
The ability to work autonomously within the codes of practice and guidelines whilst being accountable for own professional actions, demonstrating the ability to carry out the role of lead practitioner within the clinical environment.
