Job summary
As a Transforming Care Practitioner for Learning Disabilities, you will be joining a dynamic and supportive developing service with a strong emphasis on quality supervision and peer support.
Your working role includes contributing to the provision of clinical assessment, formulation, and risk management. There will be opportunities for service evaluation and development, and you will work closely with health and social care colleagues from a wide range of disciplines.
You will have a county wide role, and the base will be negotiable upon appointment.
This post will support the Trust's commitment to Somerset's Transforming Care Programme, including:
* Clinical support for the Blue Light and crisis avoidance pathway for people with learning disabilities who are at risk of being detained under the mental health act.
* Maintenance and clinical oversight of the learning disabilities dynamic support register including out of area patients.
* Liaison with emergency service and specialist forensic health colleagues.
* Specialist inpatient support for people with learning disabilities who have been detained under the Mental Health Act.
Main duties of the job
- Completing assessment and formulation with service users who have a formal diagnosis of learning disabilities and concurrent mental health problems, who may pose a risk to themselves or others. Such assessment work will involve working in a range of settings. This will involve judgments involving a range of facts or situations which require analysis or comparison of a range of options.
- Supporting the delivery of health and social care intervention/treatment options in a way that promotes equality and respects diversity and the potential need for reasonable adjustments.
- Liaising with service users, families, emergency services, and health and social care colleagues.
- Assisting with Blue Light and crisis pathway process.
- Assisting with service development work.
