Job summary
The role of the specialist peri-assessment nurse offers an exciting opportunity to be involved in a new service to improve peri-operative care for patients undergoing elective surgery. We are looking for a candidate with the enthusiasm and vision to develop newly established pathways within this service, building on the success of the existing peri-operative service so far.
The overall aim for the peri-operative service is to provide a comprehensive pathway that runs from initial referral in primary care, through to discharge, rehabilitation and long-term follow-up. This will involve complex change to services which undertake the work-up of patients having elective surgery in Somerset.
As a specialist peri-assessment nurse you will identify and begin optimisation opportunities for patients listed for surgery, manage patients and make decisions on clinical pathways. There is major emphasis on converting our waiting lists to preparation lists: getting patients ready for surgery by encouraging lifestyle changes and health promotion.The post requires established and excellent pre-operative assessment skills. It will involve working closely with lead peri-op nurses for diabetes, frailty and anaemia, as well as the other members of the peri-operative team, anaesthetists and surgeons.
The successful applicant will demonstrate evidence of professional leadership, organizational & planning skills, and appropriate motivation and enthusiasm for the role.
This role is also open to ODPs
Main duties of the job
The successful applicant will identify opportunities for health optimisation in patients who are listed for surgery, and will, in tandem with other professionals in the peri-operative service, ensure that patients are enrolled in / signposted to the appropriate peri-operative workstreams in order that any health risks / issues, that might otherwise have any adverse effect on surgical pathways, are addressed and rectified.
By doing so, the applicant will facilitate improved outcomes for patients undergoing surgery.
The role is a hybrid of on site and home-working, and the applicant must be willing to travel to other trust sites.
