Job summary

An exciting has arisen to join the Reablement therapy team for both a permanent and secondment position. The team is based in the South Somerset cluster of intermediate care pathway beds, which currently include Cooksons Court, Sherborne House and La Fontana.

Do you enjoy completing prescription rehabilitation from both Physiotherapists and Occupational therapists with a complex variety caseload during their inpatient stay?

Do you enjoy participation and running of groups such as exercise classes, mindfulness sessions and Breakfast clubs?

Come and join our friendly therapy team which consists of Physiotherapists, Occupational Therapists and Senior Rehabilitation Assistants and learn invaluable community and rehabilitation skills that support patients through their whole pathway journey.

Main duties of the job

To work as an integral member of the Intermediate Care Service providing effective support to Occupational Therapists, Physiotherapists, and any other team members

To work with patients undertaking therapy/rehabilitation programmes to support and encourage them to achieve their optimum level of independence and quality of life, as directed by team therapists

To work with patients in a range of community locations, including community hospitals, care home rehabilitation units and in service users own homes with overall supervision from rehabilitation professionals.

Essential

  • Adult/elderly rehabilitation, nursing, care worker experience
  • Post holder will be required to achieve NVQ 2/3 or equivalent in care/rehabilitation
  • Maths and English level 3 or equivalent

Experience

  • Ability to implement basic treatment plans with appropriate support from the therapist
  • A good understanding of health and social care models
  • An understanding of the physiology, psychology and organisation and function of the human body
  • Understanding of actions which can improve an individual's quality of care
  • The provision and promotion of holistic person-centred care
  • Understanding of duty of care and safeguarding of individuals
  • Understanding of patient-centred goals and how to support individuals to achieve their goals and promote independence
  • Understanding of an individual's place in society and key supportive factors
  • Previous experience of working within a health or social care setting or undertaking a caring role
