Job summary

To work as a specialist Nurse within the diagnostic pathway for children and young people in Somerset aged 5 - 17 covered by the Neurodevelopmental assessment team as well as working with Young People diagnosed with Attention Deficit Hyperactivity Disorder (ADHD) in the Children and Young People's Neurodevelopmental partnership.

Carries out assessment of cases as part of the MDT working with colleagues from different professional backgrounds within the team to give bespoke assessments depending on the age and presentation of the individual. Gives advice to other members of the MDT on nursing aspects of children and young people's presentations to inform the assessment process.

Main duties of the job

The individual will use specialist knowledge and expertise in making decisions regarding the most effective and appropriate care of children diagnosed with ADHD and associated conditions - both autonomously and in collaboration with medical staff and other professionals. They will provide effective clinical care and leadership on a daily basis for this defined case load of children.

Theindividual will work within the system of a multidisciplinary approach to patient centred care, assessment and delivery, which builds on the philosophy of greater involvement of patients and their relatives / carers by using the Trust Patient and public involvement strategy. The individual will liaise with other professionals to ensure that planned care and assessments are coordinated, effective and efficient. Promote the wider national aims of public health education.

Being responsible for individual case load of Young People having assessments for neurodiversity and those diagnosed with ADHD as well as taking on additional responsibilities as and when required by the Trust.

Communicate as required with the child, young person and carers providing sensitive clinical information in a way that takes into account the level of understanding and any barriers that may have an impact on the individual receiving and understanding the information, using appropriate visual or practical demonstrations where necessary.

To embrace the Trusts vision of candour and compassion in all communications.

Use the Trust computer system to update patient information, type and print patient contacts and send to relevant professionals and for filing in medical record systems.

Respond to emails daily. Work to promote and develop communication via the patient electronic record programme.

Lead communication with the multidisciplinary team to plan and implement individualised care programmes. To step up to the plate and be the lead professional as required.

Write precise, accurate and timely clinical records, letters, reports and statements in accordance with the data protection act (1998). Be proactive and lead on raising issues of record keeping with other professionals.
