Job summary
An exciting and rare opportunity has arisen within the Complex anticipatory Care service for a band 4 keyworker to join our team working across the communities of Chard , Ilminster and Crewkerne. This is a full time post and the successful candidate will be required to support the development of this new and exciting Neighbourhood team.
Complex Care are part of the neighbourhood team of South Somerset , working alongside health and social care teams, primary care networks and GP practices . We are leading the way in anticipatory, patient centred care, supporting those with complex health and social difficulties to reduce avoidable hospital admission , ensuring we enable people to access appropriate care and support.
The successful candidate will play a vital role in developing the team, working with a GP and senior nurse , undertaking community visits, liaising and networking with colleagues in a multi-agency approach , developing plans of care and support, whilst advocating for the wishes of the individual. You will be required to be flexible, patient, kind and knowledgeable with regards to supporting holistic care and importantly be able to work in a team under supervision and autonomously with case load management and self direction.
Main duties of the job
The main duties of the job role include:
Managing a caseload of adults under supervision of an appropriate registered professional.
Actively promote health and wellbeing with regards to long term condition management.
Undertake holistic assessment of an individual to gain understanding of needs and wishes, escalating your findings to your team and wider multi disciplinary colleagues.
Monitor, evaluate and review effectiveness of treatment plans within your scope of practice.
Use local electronic recording platforms to document patient liaison.
Undertake welfare / support visits in the community.
