Job summary
We have an exciting opportunity for a highly motivated and proactive individual to join the AMU/SDEC Department at Somerset NHS Foundation Trust as a trainee Advanced Clinical Practitioner (tACP).
The appointed trainee ACP will have a nominated Educational Supervisor, as well as a qualified ACP Mentor throughout the training program.
You will be supported to achieve aligned Faculty of Acute Medicine ACP competencies, alongside completing additional courses to support you in your practice.
Progression into a band 8a ACP role will be subject to satisfactory completion of the ACP program, including achievement of a Master's degree and accredited registration with the faculty for advanced practice.
Our SDEC/AMU department provides acute medical care to the population of Somerset (327,000). We work closely with all medical specialties, the oncology service, emergency department, primary care services and other specialties to assess, investigate and treat the full range of medical presentations.

Main duties of the job
To be responsible for the rapid assessment, diagnostics, and treatment of individuals within the Acute medicine service using an advanced level of professional accountability, autonomy, and judgement, underpinned by Masters level theory and experience.
This includes highly complex decision-making across a broad range of differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses,
To plan and manage complete episodes of care. This will be working in collaboration with senior medical colleagues and others when required. Delegating and referring as appropriate, to ensure timely, effective management to optimise health outcomes, in line with the evidence base and nationally recognised best practice.
To play a key operational role and maintain a pro-active approach to managing, updating and leading clinical pathways.
To promote and implement service and policy development and redesign, impacting beyond own area and other disciplines, informed by current best practice and evidence-base, in accordance with values-based care.
