An exciting opportunity has arisen within our Community Mental Health Service to work within one of our teams, based in Bridgwater. We are looking for an enthusiastic practitioner who is keen to be part of the exciting transformation of Mental Health Services in Somerset .
We are a trailblazer Trust for the Community Mental Health Framework for Adults and Older Adults, and have received significant investment to transform all our community mental health services and have recently been awarded Mental Health Trust of the Year.
There are currently two posts available, one full time (37.5 hours) and one part time (22.5 hours).
Main duties of the job
The post holder will deliver a high quality, comprehensive mental health service for patients with severe and enduring mental health difficulties within the Community Mental Health Service. The role involves managing a defined caseload, with a focus on working with individuals who experience a complex range of needs and difficulties, and supporting their significant others. There is also a requirement to take part in depot and wellbeing clinics on a rota basis.

The post holder will deliver a high quality, comprehensive mental health service for patients with severe and enduring mental health difficulties within the Community Mental Health Service. The role involves managing a defined caseload, with a focus on working with individuals who experience a complex range of needs and difficulties, and supporting their significant others.
Acting as a key member of the multi-disciplinary team, the post holder will provide assessment, psychosocial, evidence-based interventions using a collaborative, recovery approach and will beskilled at positive risk management. The post holder will contribute to the clinical leadership of the team, providing supervision for other team members, as agreed with the Team Manager, and participate in team and service development and evaluation. Liaison with a wide range of agencies is central to the role.
The post holder must hold a full driving license and have permanent access to own transport and have appropriate insurance.
