Job summary

We are seeking an Advanced Nurse Practitioner with ambition and drive to join our thriving, forward-thinking and supportive team.

Meadow Ward, Williton Community Hospital is a centre of excellence for stroke recovery and end of life care. We are a 20 bedded unit in beautiful West Somerset, and pride ourselves on our holistic approach to patient, family and staff care.

The Advanced Nurse Practitioner is a nationally recognised role, and these posts offer an exciting opportunity for senior professionals with extensive clinical experience and independent prescribing.

We are looking for the right person to complete our clinical team. We will support you to complete your training to become an Advanced Clinical Practitioner.

The role will be Monday-Friday with flexible daily working hours according to need.

Main duties of the job

The Advanced Nurse Practitioner will support the wider inpatient clinical team, by providing assessment and management of inpatients during admission, discharge and episodes of clinical concern. Ensure all care is relationship-centred and length of stays minimised.

Responsibility for patient/client care, treatment & therapy includes:

Prescribe appropriate treatment with a clear plan for review and subsequent discharge. Undertake on-going holistic assessments of patients medical conditions including individualised physical, mental health and social care needs assessment and actions Develop management plans that are person-centred and include key medical information such as resuscitation status and Treatment Escalation Plan Improve the quality of the care experience by identifying and meeting the individual clinical needs Provide specialist advice on patient care to the healthcare team, taking into account all aspects of clinical governance, and act as a specialist resource within the ward and other areas around the Trust

  • Registered first level nurse
  • Relevant nursing/health degree/diploma level supplemented by minimum PG dip (120 credits) plus MA/MSc or working towards in a relevant subject or demonstrable extensive experience in the relevant specialty
  • Independent prescribing

Desirable

  • Clinical supervision training and experience

Knowledge

Essential

  • Management of patients with long-term conditions
  • Management of patients with complex needs and frailty
  • Accountability of own role and other roles in a nurse-led service

Desirable

  • Able to identify determinants on health in the area

Experience

Essential

  • Clinical Leadership - This should include experience at a senior clinical level, minimum of a band 6, demonstrating the appropriate competencies for the role
  • Experience of working within a consent and capacity framework
