Job summary

We are looking for a Project Manager to help develop and establish new Stroke Association Key Worker roles in our existing Integrated Community Stroke Service in Somerset.

This service is critical to empowering people, and those important to them, to live well after having a stroke. You'll be working closely with clinicians across acute and community teams to make this happen.

You'll be someone with demonstrable experience of managing and delivering a project with excellent organisational, analytical, communication and problem solving skills. You'll be able to deliver projects using project management techniques, working and communicating across organisational boundaries, whilst keeping to time and budget constraints and maintaining quality in line with National guidance.

Main duties of the job

  • Manage all elements of the project, facilitating and monitoring progress and providing timely and accurate reporting of key data and regular reporting to the NHS England Squire Lead, Somerset ICS and Somerset FT Stroke Governance Group.
  • Develop project governance using appropriate project management principles
  • Develop and manage the detailed project plan, identifying the tasks to be completed within appropriate timescales, the task owner and the dependencies. Make adjustments to the project plan if there are any changes identified by project steering group.
  • Escalate to the Operational Manager if key targets are not going to be met, with suggested actions for their resolution.
  • Work collaboratively with the Neighbourhood and Medical service groups, and the Stroke Association, as partners in developing and delivering the key worker role, this will involve travelling across Somerset as required.
  • Identify key contacts and develop strong working relationships with internal and external stakeholders, including local community health partners.
  • Communicate to, and gain support from, stakeholders, providing guidance, advice, and signposting on complex implementation issues.
  • Capture and share outputs and learning from the project to contribute to the development of the Stroke recovery service, including production of robust business case for recurrent funding.
