We now have an exciting opportunity for an energetic, enthusiastic, and skilled, mental health nurse/practitioner to work within our CMHT for Older People in South Somerset, based either in Chard or Yeovil.

The post holder will benefit from high levels of support from their Integrated Service Manager, as well as support from an effective and experienced peer network, at both a local and county wide level.

We provide high quality care for people with severe and enduring mental health needs by working actively in partnership with our service users, their carers, the Local Authority, other statutory services and local voluntary groups.Acting as a key member of the multi-disciplinary team you will use your skills and experience to work collaboratively with a caseload of service users, their family members and carers to develop recovery focused care and support plans that promote and enable independence, reduce social exclusion and isolation and maintain their quality of life.

The successful candidates will be supported through a comprehensive induction and will receive regular supervision to support your progression through learning and development opportunities related to their role.

Main duties of the job

The successful post holder will be responsible and professionally accountable for a caseload within the community, working with individuals and their significant others who have multiple and complex needs. Acting as a key member of the multidisciplinary team, the post holder will provide assessment , evidence based interventions using a collaborative recovery approach and will be skilled at positive risk management.

The post holder will contribute to the clinical leadership of the team, providing supervision for other team members and participate in team and service development.

It is expected that the post holder will provide a high standard of care and treatment for patients and work with other members of the team and other agencies to best meet the patients' health and social needs.
