Job summary

This post offers a new and exciting opportunity to work within West Somerset Primary Care Network, covering surgeries in Minehead, Williton, Dunster and Dulverton.

You will link in with the neighbourhood hub; being part of our our 'Living Better' Multidisciplinary team: Complex care nurses, a complex care GP, Care home lead, large team of social prescribers including our health coaches and our pharmacy team. You will be supported by the MDT and join our development days.We hold team development at the heart of what we do; along with a healthy prescription of humour, beautiful scenery, and sea air, why not come and meet the team and consider applying?

Part of the role will be directly involved in assessing patients referred from Primary Care (this will include adults of working age and older adults), followed by identifying and facilitating appropriate support/treatment for them. For most people this will mean remaining within Primary Care - supported either by the Primary Care Liaison for brief psychological interventions, the Open Mental Health Alliance or by Talking Therapies. For some, with more complex needs, it will involve facilitating a smooth transfer into Somerset Foundation Trust Secondary Mental Health services.

You will also receive training opportunities for further development, formal monthly supervision, and an annual appraisal. Full/Part time hours considered.

Main duties of the job

To guide assessment, formulation and treatment planning for people within Primary care with a mental health need, this will include people aged 18yrs +. For most people this will mean remaining within Primary Care - supported either by the Primary Care Liaison for brief psychological interventions, the Open Mental Health Alliance or by Talking Therapies. For some, with more complex needs, it will involve facilitating a smooth transfer into Somerset Foundation Trust Secondary Mental Health services.

To provide liaison into Open Mental Health and wider Community Mental Health Services.

To participate actively in team meetings, PCN MDT's and other forums asappropriate in order to secure best possible clinical outcomes.
