We are looking for an enthusiastic Band 5 Physiotherapist to join our Community Rehabilitation Service at West Mendip Hospital in Glastonbury, Somerset.
We are an integrated team of Physiotherapists, Occupational Therapists, Assistant practitioners and rehab assistants , working with the patients over the age of 18 in their own home, providing assessment and treatment to reach patient agreed goals. We also run falls clinics across the Mendip Neighbourhood and have recently developed a new outpatient Neuro clinic with our colleagues in Mendip.

Main duties of the job
- To provide high quality patient-centered care and specialist rehabilitation to adults referred to the Rehabilitation Service.
- Participate in the development of the Rehabilitation Service in the community and within the Community hospital bedded environments, where there are teams supporting hospital inpatient units
- To provide advice regarding the rehabilitation management of patients to families, carers and other members of the MDT.
- Responsibility for a caseload of patients within both the community and/or the community hospital bedded environment.
- Carry out duties autonomously but with the ability to seek support and informal and formal supervision from Senior Therapists within the teams.
- Alongside Senior clinical staff and Clinical Educator, give support, guidance and training to Rehabilitation Assistants, Assistant Practitioners, Apprentices and Students on placement within the team.

- To be responsible for organising, prioritising and planning your own caseload to meet service and patient priorities, readjusting plans as situations change/arise.
- To ensure that your own practice, and that of the staff under your supervision, meets the required professional standards of Physiotherapy.
- To ensure your own ongoing personal/professional development.
- To comply with the Trust and departmental policies and procedures
- To be professional and legally responsible and accountable for all aspects of your own work including the management of patients in your care.
- To ensure a high standard of clinical care for the patients under your management, and support more junior staff to do likewise.
- To undertake a comprehensive assessment of patients including those with diverse or complex presentations.
- To use clinical reasoning skills and manual assessment techniques to provide an accurate diagnosis of their condition and agree patient centred goals with patients and carers.
- Formulate and deliver an individual physiotherapy treatment programme where appropriate. Use of Care plans based on evidence-based practice and treatment options using clinical assessment, reasoning skills and a broad knowledge and understanding of treatment.
- To carry out assessment and treatments of a range of conditions with moderate physical effort on a daily basis.
- To clarify and explain the patients condition to them and/or their families to enable the formulation of realistic agreed treatment goals and plans. This may sometimes involve the breaking of unwelcome news to the patient and/or their carers.
- To deal sensitively with patients, carers and families who may have high levels of anxiety as part of their complex presentation.
- To comply with the Trusts Manual Handling policy and local therapeutic handling guidance at all times.
- To be responsible for maintaining accurate and comprehensive patient treatment records in line with Trust procedures and Chartered Society of Physiotherapy (CSP) standards of practice.
- To write concise, informative reports detailing treatment outcomes and levels of disability for medical staff, employers, solicitors and Education Health Plans
- To train, supervise and performance manage more junior staff including technical instructors and therapy support practitioners.
- To communicate effectively with patients and their parents, guardians or carers, to maximise recovery potential, and to ensure understanding of both conditions and treatment.
