Nursing Associate / Assistant Practitioner, Band 4

The East South Somerset District Nurses are looking for two dynamic Band 4 Nursing Associate / Assistant Practitioner to join the teams who work across the Yeovil and Wincanton Neighbourhood.

We visit patients in their own homes and work along side our colleagues from the integrated teams, including IRT, OPMH, GPs, Complex Care Team Palliative care teams, Adult Social Care, village agents and Hospital at Home.
