Job summary

We are looking for a band 5 Stroke Community Nurse or band 4 Trainee Nurse Associate to join the service at 30-37.5 hours per week. This is an exciting role working out in the community visiting patients within their own homes to support their transition after what can often be a life changing stroke. The role involves assessing patients and developing care plans, delivering care and support, information and advice to patients aiming to prevent a further stroke or hospital readmission. You will become confident in reviewing stroke medications, continence and facilitate patients to manage their own blood pressure and make lifestyle changes.

Main duties of the job

You will be required to work their hours over 7 days, including out of hours, weekends and bank holidays, to meet the needs of the service. The Early Supported Discharge Service enables the accelerated discharge of Stroke and Acquired Brain Injury patients usually to their own home or normal place of residence from district or community hospitals.

  • To join the Early Supportive Discharge Team, being based at hubs across the county.
  • To act as keyworker for patients.
  • To work as an integral member of the Health Care /Rehabilitation Service providing effective support to Patients and Carer's as part of a team of Occupational Therapists, Physiotherapists, and any other team members.
  • To work with patients undertaking therapy/rehabilitation programmes to support and encourage them to achieve their optimum level of independence and quality of life
  • To work with patients in a range of locations, including hospital departments such as wards, rehabilitation unit, outpatients, and the community.
  • To be responsible for own personal safety, have knowledge of policy. Safeguard possessions and all equipment issued by Somerset Primary Care Trust.
  • To manage own time effectively demonstrating flexibility to meet the service requirements across the county.
  • Must be a car driver with a valid driving license or have access to transport with appropriate business insurance.

CLINICAL RESPONSIBILITIES:

  • To maintain a high degree of individualized patient centered care being responsible for the assessment, implementation and evaluation of care for patients throughout the ward. This will be achieved by devising care plans/ completing water low assessments / Manual handling assessments / Risk assessments /Observation charts and Referrals in-house and interagency forms.
  • To be responsible and accountable for ensuring care plans are amended as patients care needs dictate and evaluated on a daily basis and when there is change within the treatment plan. Assessment of wound care & pressure care will be carried out and implemented in line with manual handling guidelines.
  • To ensure that patients are treated with dignity and respect, to ensure that high quality of care is given. Ensuring pressure area care/ elimination of body fluids are assessed frequently over a 24 hour period to maintain their comfort by ensuring regular change of position using the minimal handling guidelines.
  • To personally administer all relevant forms of direct patient care, also to be responsible for the supervision of junior staff on a daily basis within the Guidelines of the Professional code of accountability.
  • To obtain additional clinical competencies and knowledge in a variety of skills; e.g. male catheterization/ venepuncture/Intra-venous drug therapy/ Anticoagulant test /Cardiac defibrillation/Intra-Venous Cannulation/ Electro cardiac graph and Bladder Scanning.
  • To promote a safe environment within the unit ensuring staff are trained and updated in health and safety, fire regulations, COSHH, infection control, back awareness and cardiac resuscitation.
  • To initiate and participate in the development of education programs NVQs and mentorship, facilitating standards of care to junior staff, carers and patients
  • To support and assist in audits / monitoring / collecting data / transferring data to other sources; in collaboration with the ward manager & Matron. To maintain Quality Assurance initiatives by playing an active role with self checks on equipment and attending annual training sessions.
  • To contribute to the quality agenda, including Evidence Based Practice, Audit, Clinical Governance and Clinical Effectiveness.
  • To participate in an annual appraisal with their ward manager so that an agreed training programme can be developed. This will endeavour to maintain their clinical expertise and give them the responsibility for their own Personal Development Plan
  • To initiate the appraisal process with junior staff nurses and HealthCare Assistants to continue the evaluation process & education needs of staff development as directed by the Ward Manager.
  • To contribute to Staff induction program and the continuing education of perceptorship and newly qualified staff /new Trust staff / student nurses/ cadet Nurses and work experience students. To promote an informative educational environment this will aid effective communication between the two parties.
  • To provide advice/support to team members and contribute to, and promote their personal and professional development, as directed by the Ward Manager.
  • To attend meetings with team members and other professionals, to ensure good liaison within ward, to attend at least 6 ward meetings annually.
  • To access and implement as necessary all Somerset Partnership Trust policies/protocols and guidelines
  • To work in partnership with Multi-disciplinary team and other medical staff.
  • To advise patients and carers on health care issues and support them in times of stressful and emotional events which are frequently emotionally demanding to the nurse and demanding on their time.

  • To be able to take responsibility of the Ward when the Ward Manager is not on duty. To delegate nursing staff, prioritise, make decisions and communicate effectively with all levels of staff.
  • To keep Professional knowledge up to date, research recent developments and implement where appropriate within the Clinical setting.
  • To provide evidence based care and promote in others, cascading information learnt on courses to staff, patients and carers, and to maintain high standards of care in a professional manner.

MANAGERIAL
