We have a Full time Band 6 Occupational Therapy role available within the Burnham-On-Sea area of our Community Rehabilitation Team - .
We are a Community Therapy team, working with Physiotherapist, Occupational Therapists and Pharmacy technicians in Somerset. Our aim is to help the local population maximise their abilities to improve their quality of life.
We are changing as we move to an innovative neighbourhood model as part of an Integrated Care System. We have an emphasis on prevention, early intervention at the initial point of need and long term self-management to enable patients to live better lives in their home environment. We see people in a host of settings, led by their goals but to include: their own homes and local community, our community hospitals and our local care homes.
You will play an integral part of the Community Rehabilitation team, managing a complex community caseload assessing and treating patients either in a variety of residential settings. These patients may present with a broad range of conditions including orthopaedics, frail elderly, post-surgery and neuro long-term conditions.

The Post Holder will:- Provide a high standard of Occupational Therapy within the Rehabilitation services. Participate in the development of the Rehabilitation Service in the community and the community hospital (where appropriate). To provide advice regarding the rehabilitation management of patients to families, carers and other members of the MDT. Provide support, guidance and training to Assistant Practitioner, Rehabilitation Assistants and students. Responsibility for a caseload of complex patients within both the community and the community hospital (where appropriate). Act as Clinical Supervisor and undertake Managerial Supervision, including Appraisal, for Band 5 Occupational Therapists and Rehabilitation Assistants.
To use advanced assessment and clinical reasoning to predict achievable outcomes of treatment and to recommend the best course of intervention, developing comprehensive treatment plans, in conjunction with other professions and agencies. To be responsible on a daily basis for the allocation of clinical work to the Rehabilitation Assistants To manage clinical risk within own patient caseload and take action to effectively manage identified risks To play a key role in planning and implementing discharge from bedded environments in conjunction with other professionals and agencies. To communicate effectively and work collaboratively with medical, nursing, social services and therapy colleagues to ensure delivery of a coordinated multidisciplinary service. This will include providing specialist/ expert advice to case conferences, discharge planning, patient reviews and admission prevention. To be responsible for organising, prioritising and planning your own caseload and that of the staff under your supervision within the Rehabilitation service To manage clinical time effectively by caseload prioritisation to meet patient need, balancing this with other patient related activities and professional responsibilities, both planned and unpredictable demands To ensure that your own practice and that of the staff under your supervision meets the required professional standards of Occupational Therapy. To comply with the Trust and departmental policies and procedures.AnalyticsTo be able operate appropriate Information and Digital Technology systems tocontribute to record keeping and data analysis, under guidance from Trust Policy.Participate in data analysis/clinical audits as required.
