Job summary

Are you a Registered Mental Health Nurse, Social Worker, Occupational Therapist?

We are seeking registered, motivated mental health professionals to work in our expanding Home Treatment Team (previously known as the Crisis Service) based in one of our 4 teams across Somerset (Yeovil, Taunton, Wells, Bridgwater).

You will act as a key member of the multi disciplinary team, liaising with statutory services and other agencies. You will be part of a dynamic team, who are responsible for ensuring rapid community assessments of mental health and risk. You will have sound assessment skills and provide brief interventions to clients as an alternative to admission.

Main duties of the job

We recognise that recovery from acute mental illness is best met within a persons own home, so we strive to provide safe, intensive mental health treatment and intervention at home as an alternative to admission to psychiatric hospital.

You will be a key member of a skilled Multi-Disciplinary Team who will offer a wide range of psychological, medical and social interventions to support recovery

As well as working with a clinical caseload and their families, the home treatment teamgatekeep admissions to and facilitate early discharge from psychiatric inpatient settings

This role will involve flexible working across a 24-hour shift rota (days and night shifts) in a 7 day a week service.

  • RMN, Social Work or Occupational Therapy Qualification
  • Current registration with relevant professional body
  • Evidence of post qualifying professional development in an acute setting

Knowldege and Experience

  • Significant post registration experience in multi-disciplinary community mental health services
  • Experience of managing risk of clients in crisis
  • Experience of assessment, care-plan, development and evaluation in conjunction with clients and carers

Desirable

  • Experience of mental health difficulties
  • Good knowledge of resources and working practices across health and social care
  • Ability to be innovative in practice
