Job summary

We are excited to say that here in Somerset, service users, commissioners and providers across primary care, secondary care and the third sector have come together to co-produce what we collectively believe is a bold and radical redesign of a model of mental health care that has largely remained unchanged for 20 years. The planned Community Rehabilitation service will integrate with our innovative core community mental health transformation, and develop a joint service approach, in supporting the rehabilitation of Somerset patients building on existing service developments.

This exciting role will drive the service forward and manage the Community Rehabilitation service along with the Inpatient Rehabilitation ward and the VCSE offer within our planned new service. SFT and VCS partners have successfully partnered to jointly deliver transformed community mental health services to the population of Somerset. This allows people to access a wide range of services and support to meet their mental health needs, including debt and employment advice, volunteering opportunities, community activities and exercise as well as access to specialist mental health services. The aim is to add this new joint rehabilitation service to the number of service options within the county that really puts the person at the centre of our MH offer.

Main duties of the job

  • Operational Manager leading the services of Community Rehabilitation service & patient flow between Willow rehabilitation ward and the community rehab service. Providing direct line management to staff across a range of professional backgrounds in the community rehabilitation service.
  • To develop working relationship between the community rehabilitation service, and the clinical team on the rehab ward, Willow, ensuring the services work closely together with wider MH services/stakeholders to support a collaborative approach to the rehabilitation of patients into the local Somerset community.
  • To manage the working relationship, and, the contract with VCSE Rehabilitation service
  • Working alongside the CMHS and LD sub directorate management team will co-ordinate, lead and develop the delivery of the service(s)
  • Will undertake clinical duties and may be required to maintain a clinical caseload.
  • Will be expected to participate in the Manager On-call rota.
