Job summary
We are looking for a dynamic and inspiring Occupational Therapist with a special interest in palliative care to join the Medical Occupational Therapy (OT) Team.
Due to some investment in this specialist area this is an exciting new role at Band 6 level. The post holder will contribute to increasing our knowledge and integration with the services that support the person's treatment and recovery during the last 1000 days. The post will include educating colleagues to be able to provide a consistent approach from OT to those who need a palliative service, including at times, supporting a patient to return home for End-of-Life care with or without fast-track funding (CHC). There will be an element of the role where it will be appropriate to support the patient to transition from the acute setting to the community.
The post is a static position giving opportunity to develop the service, gain specialist skills and knowledge and build excellent working relationships with the extended multi-disciplinary team within the acute hospital as well as establishing knowledge and relationships with community services.
As this is a new role at Musgrove Park Hospital, there will be an element of exploration and development of the role which will be supported by the team leader. This is an exciting opportunity for anyone who has a dual interest of service development and a desire to support high quality palliative care.
Main duties of the job
To deliver and development an effective and high quality specialised Occupational Therapy Palliative Care Service. This will include provision of specialist education and advice to patients, their representatives and colleagues.
Part of the role will be supporting colleagues to be able to provide a consistent Occupational Therapy response to those who need a palliative approach, including at times, supporting a patient to get home for End-of-Life care, with or without fast-track funding.
The post holder will supervise junior members of the team as required, as well as supporting Occupational Therapists, students and multidisciplinary staff working within the medical team within the workplace. The post holder will deputise for the Advanced Clinical lead or Team Leader undertaking any specific clinical and operational responsibilities in their absence.
The post holder will be responsible for his or her own Continual Professional Development (CPD)

To provide specialised occupational therapy expertise in the assessment treatment and management of patients with complex physical and psychological conditions. This will include the use of manual occupational therapy techniques and high level analytical, reasoning and risk assessment skills to select appropriate intervention from a range of options.
To complete a comprehensive assessment for a designated caseload, addressing occupational performance and skills deficits and cognition, enabling the client in areas of self-maintenance, productivity, and leisure.
To plan and implement individual and/or group interventions, in collaboration with the client, using graded activity to identify and achieve therapeutic goals.
To monitor, evaluate and modify treatment for clients with multiple needs to measure progress and ensure effectiveness of intervention.
To demonstrate and apply a broad level of understanding of the effect of disability and recommend adaptations to the clients physical and social environment provided by the equipment loan service.
Monitor, evaluate and modify interventions for patients to measure progress and ensure effectiveness of therapy using agreed outcome measures and sharing the results with appropriate partner agencies to facilitate smooth transfers of care.
To assess and provide appropriate assistive and adaptive equipment to facilitate safe and timely discharge for patients from hospital or when assessing in home environment, in a manner which reflects cost effective use of resource, with appropriate application of relevant eligibility criteria. To refer for onward assessment where necessary.
To be professionally and legally accountable for all aspects of own clinical practice, including the management of patients.
To take delegated responsibility from the other senior occupational therapists for managing patients with specific conditions and be responsible for delivering an individual occupation therapy treatment programme based on sound knowledge of evidence-based practice.
To accept clinical responsibility for a designated caseload of patients, and to organise this effectively and efficiently with regard to clinical priorities and use of time.
To represent occupational therapy and/or individual patients at multidisciplinary team (MDT) meetings, to ensure the delivery of a coordinated multidisciplinary service, and integrate occupational therapy into the treatment programme. This will include discussion of patient care, patient progress and involvement in complex discharge planning.
To respond appropriately to a range of unpredictable circumstances, in case load or working environment.
To provide spontaneous and planned advice, teaching and instruction to relatives, carers, and other professionals, to promote understanding of the aims of occupational therapy, and to ensure a consistent approach to patient care.
