Job summary

Who are we?

We are a nationally recognised intermediate care service that provides support to keep people stable, well and independent. We support people to avoid hospital admission (called rapid response) and we support people to leave hospital (called supported discharge). Our goal is to support people to recover at home wherever possible, but we also provide support to people in a number of different reablement bedded units across different areas of Somerset.

We are nationally recognised as a mature integrated service - our intermediate care service was designed in April 2020 with integration at it's core. Our joint Local authority and NHS funding has enabled us to become a co-designed team of therapy staff, social workers, nurses, reablement support workers, community agents, care providers and care homes. Every member of our team has a unique background and a unique contribution to delivering and shaping recovery and reablement services. As 2021 HSJ Partnership Award finalists, we pride ourselves in the collaborative work that we do locally with our neighbouring organisations as well as the work we are involved in nationally.

  • Reablement unit registered occupational therapist (Band 5 & 6) - West Mendip, Somerset
  • Reablement Unit assistant therapists - Taunton, Somerset
  • Pathway 1 registered therapists (band 5 & 6) - variety of Somerset locations
  • Rapid Response registered therapists (band 6)- Taunton & Mendip
  • Reablement Unit Therapy Lead roles (band 7) - Variety of locations

Intermediate Care Services in Somerset are fast growing. Currently we are supporting c.600 people across the county of Somerset. Our county wide service enables us to be able to offer opportunities in a variety of locations across Somerset. We have opportunities for you to work in people's homes, in reablement units or a mixture of the two. Whatever it is, we have an opportunity for you!   • Supervision and appraisal experience
  • Evidence of advanced clinical reasoning
