Job summary

Are you an experienced Therapist working at Enhanced Practitioner level or a therapist wanting to develop into an Enhanced Practitioner role?

This is an exciting new opportunity for an therapist to join the Neighbourhood teams as an Enhanced Clinical Practitioner. You will either be able to demonstrate your enhanced clinical knowledge and skills or willing to develop towards this, with an interest in long term conditions.

The Neighbourhoods are a supportive and welcoming service and although this role is based in West South Somerset, your knowledge and skills may be utilised across the county to support patients with long term conditions.

Main duties of the job

The post holder will be a skilled, autonomous practitioner working as a core member of the Rehabilitation service, Acute therapy team or Neighbourhood and wider multidisciplinary team to deliver high quality care to patients. They will possess the skills, competence, expertise and clinical decision-making abilities required to plan, co-ordinate and carry out enhanced rehabilitation treatment for patients with long term conditions within the community setting.
