Job summary

We are looking for two Integrated Neighbourhood Project Manager to join the team. One on a permanent basis and one on a 18 month Fixed Term/Secondment.

The Integrated Neighbourhoods Project Manager will provide transformation support and project management for the Integrated Neighbourhoods programme of work. This is a key priority for the new Cambridgeshire & Peterborough Integrated Care System (ICS) and is being delivered via the Cambridgeshire South Care Partnership.

You will support the development and delivery of the Integrated Neighbourhood, are based around Primary Care Network footprints, and include a wide range of stakeholders from different sectors. This role will facilitate engagement, positive relationships and bring together all partners who work with the identified Neighbourhood population.

The Project Manager will work closely with the Primary Care Network, clinical teams and other system partners to review their population health data and support public engagement to determine how local citizens can better shape their health and care services.

Main duties of the job

The role will be responsible for supporting the service redesign and delivery of specific projects within the Integrated Neighbourhood, coordinating activities to facilitate the successful delivery of the Integrated Neighbourhood objectives and a more person centred and proactive approach to health and wellbeing.

In addition to supporting a specific Integrated Neighbourhood(s) the Project Managers will support the delivery of the integrated working improvement projects developed by North and South Care Partnerships. This programme of work aims to make operational changes to improve cross-organisation working and remove the barriers to integration.

This role requires close partnership working with clinical leaders, the acute sector, community services, health commissioners, local authorities, voluntary & community organisations, and the public to support the Integrated Neighbourhood development and service design. It requires the ability to support the translation of a clinical vision into a coherent operational model and project delivery.
