Job summary
We are looking for a highly motivated individual to join our team in Cancer Support.
The role is challenging but varied and requires excellent organisational and communication skills. You must have the ability to use your own initiative, showing attention to detail and working well under pressure, independently and in a team. In return, you will enjoy a varied role supporting patients affected by cancer, working closely with our clinical teams.
This exciting role is part of the Personalised Care agenda, responsible for delivering Holistic Needs Assessments to patients and supporting them along their pathway. This post is one of 8 Navigators within the wider Macmillan CUH Cancer Support Team.
Main duties of the job
Support the Macmillan CUH Cancer Support Team to implement the Survivorship Initiative including Personalised Care (formally known as Macmillan Recovery package).
Assist the Macmillan CUH Cancer Support Team and healthcare professionals to implement Holistic Needs Assessment and End of Treatment Summaries.
Support people to navigate the complex health and social care system.
Act as a main point of contact for patients, carers and the wider healthcare teams, ensuring the joining up of services, coordination, information and communication between all creates a seamless experience of care.
