Job summary

The West Suffolk Foundation NHS Foundation Trust has a four year post rotating in Emergency Medicine to Anaesthetics, Critical Care and Acute Medicine.

  • This post is ideal for doctors who wish to obtain specific competencies to enable entry to formal training at ST3/4 level or by the CESR route, DREEM and Article 14.
  • The West Suffolk Foundation Trust will provide middle grade position and experience in Emergency Medicine and junior positions in rotational specialties, each specialty component lasts 6 months.
  • Clinical supervision, competency assessment and education are part of the package; each doctor will have an Educational Supervisor.
  • The appointment is suitable for middle grade, clinical fellows and LAT doctors in Emergency Medicine.
  • Previous experience in Emergency Medicine is essential.
  • There is an intense and fully supported education and training program with generous provision for study leave with full funding.
  • The department is a 24/7 designated Trauma Unit dealing with the full spectrum from critical illness, the acutely ill, major trauma and care for the ambulatory stream for a population of 225,000
  • The Emergency Department managed 79,953 patients in 2019.

Main duties of the job

  • You will be part of the multidisciplinary clinical team as middle grade in the Emergency Department and your principal function is to:
  • Provide high quality care, knowledge, and skill for the patients either directly or by supervision and floor management of the junior clinical staff
  • To maintain the standards of the College of Emergency Medicine and the West Suffolk Hospital NHS Foundation Trust
  • To assume clinical responsibility for patients brought to the department by the emergency services, self-referring or referred by GP
  • You are expected to deal with unexpected emergencies including those resulting from infectious diseases like COVID-19
  • To be part of Trauma Team co-ordinated management of major trauma
  • To be immediately available for opinion, guidance and advice on any patient seen by a junior clinician in the ED at any time during the rostered duty shift
  • To assist in conducting follow-up review of patients where considered appropriate by the Consultants
  • To contribute to the production of departmental clinical management guidelines in line with evidence-based best practice
  • To attend in the event of a hospital Major Incident call if available and off duty
  • Maintain patient confidentiality
  • Identify and act appropriately upon Safeguarding issues in children appropriately

Job responsibilities

This post is for doctors who are currently in a middle grade position in Emergency Medicine who need to complete specific ACCS competencies to enable their career progression.

The applicant should be aiming to become a Consultant by entering formal training at ST3/4 onwards or by the CESR route and Article 14, or the Defined Route of Entry into Emergency Medicine (DREEM). This post would provide a middle grade position plus experience in Emergency Medicine and a junior position in the rotational specialties. It will provide specific rotational posts, clinical supervision, competency assessment and education.

This is a new middle grade rotation position as Senior Clinical Fellow in Emergency Medicine which rotates to six month positions in Anaesthetics, Critical Care Medicine and Acute Medicine. There are eleven posts in total comprising eight posts in the Emergency Medicine and three in the specialties at any one time. This enables both the provision of high quality middle doctor staff to the Emergency Department and the rotation competencies for their career progression.

The appointment is suitable for those with middle grade Emergency Department experience as Specialty Doctor (old Staff Grade), Clinical Fellow, LAT STR or completed ST1 or ST2. Applicants from Anaesthetics, ITU, Acute medicine backgrounds or completed GPSTR training will be considered if proceeding to a career in Emergency Medicine.

The post duration is up to 48 months in 18 month rotational blocks. The rotation will maintain pay but the banding may change according to the individual departmental rota.

Two of the Clinical Fellows have finished this rotation and successfully entered an ST4 position. Most of our Clinical Fellows pass their MRCEM within two and a half years of starting this post. There is an intense training program associated with this post.

Educational Supervision and Career progression

The appointee will have an Educational/Clinical Supervisor for the duration of the rotation and a designated clinical supervisor for each post. The appointee will be expected to maintain a portfolio of evidence of competencies, work place-based assessments, training undertaken with supervisions and a personal development plan. This will be in keeping with the requirements of the e-portfolio and Curriculum of the College of Emergency Medicine.

Career progression will be a priority and guidance can be provided by the Emergency Medicine Trust College tutor and Educational Supervisor. Associate or Membership of the College of Emergency Medicine will be expected. E-portfolio applications will be supported.

Educational Arrangements

There will be a competency plan based on the doctors requirements at ACCS CT1/2/3 level. Training may be catered to MCEM examination requirements. Our teaching programme has been very well received and we have an increasing number of candidates who have gone into or completed higher specialist training with us.

There will also be the opportunity to deliver teaching as part of the regional training programme.
