Job summary

As a part of the busy inpatient diabetes specialist nurse team, and supported by senior diabetes specialist nurses, you will collaborate with clinical colleagues to offer specialist knowledge on the management of diabetic patients.

You will provide education for patients and their carers, empowering individual in their diabetes management.

Supporting nursing colleagues across the trust, in providing education and information for patients and/or their carers in the management of diabetes.

Main duties of the job

Work as part of a specialist team.

Daily review of new referrals, including patients within the emergency department.

Provide face to face education programmes to patients and their carers.

Daily review of existing inpatients

Support all grades of ward nursing staff in relation to diabetes nursing, and ensure a safe framework for practice is in place and maintained

Excellent communication skills are required to facilitate effective management methods are clear and understandable.

Ensure each patient is given an individual management plan.

Work autonomously, being supported by senior team members.

This is a Fixed term Contract/Secondment for 9 Months.

To work in collaboration with other health care professionals to develop new and innovative tools to improve patient care

To work in collaboration with patients and their carers to develop individualised care plans providing knowledge and support enabling them to self-manage their diabetes

To provide support for all grades of nursing staff in relation to diabetes nursing and to ensure a safe framework for practice is in place and maintained

To work with patients in a variety of settings (i.e. clinics, wards and community) providing a service that is sensitive to the needs of the individual

To have specialist knowledge regarding the actions of both oral and injectable medications used to treat Diabetes, ensuring safe practice in the hospital setting

To act as specialist resource

To deal with distressing/emotional circumstances related to patients/relatives and carers

To provide support and advice to patients and their carers with regard to their individualised requirements

To provide support/advice to nursing and medical colleagues in order to promote best practice with regards to diabetes management

To ensure own knowledge is evidence based and up to date, working in line with the Diabetes Team current thinking

To access electronic referrals, review referrals and prioritise them according to need

To assess patients who are referred and ensure that every patient is reviewed within a timely manner To work autonomously with supervision from Senior Diabetes Specialist Nurse Team Communication & team working:

Establish effective working relationships with other health care professionals whose role links to the diabetes e.g. dietitian, surgical and medicine department, pharmacy and primary care, nurses, doctors palliative care team, etc.

Ensure nurse specialists achieve clear communication between secondary and primary care regarding management plans for diabetes patients.

Lead on establishing effective communication across professional boundaries within the trust in regards to the introduction of new initiatives or alterations in protocol or policy development or referral guidelines.

Collaborate with consultants and ward managers to ensure that clinical staff within the trust are supported through education, advice and clinical protocols to effectively manage patients diagnosed with diabetes.

Establish and maintain effective communication networks and working relationships with personnel within and outside the organisation.

Empower patients to make informed choices regarding lifestyle changes, sign-posting to structured education programmes.

Support nursing colleagues in providing education and information for patients and/or their carers in the management of diabetes.

Management responsibilities:

To work in partnership with diabetes team in furthering services for the benefit of patient care To respond to complaints and risk management issues relating to the in-patient service Develop and implement monitoring systems to measure the effectiveness of the service

To provide highly specialised clinical advice re Diabetes management

To work closely with link nurses to provide support, ensuring new developments are effective

Education:

Organise, evaluate and deliver educational programmes for in-patients, referring to NICE guidelines and the National Service Framework

To participate on internal and external teaching programmes and to maximise teaching opportunities

To participate in active health promotion for patients, ensuring a clear understanding of diabetes and its treatments and discussing any anxieties/concerns with appropriate physicians

To identify training needs and to develop specialised diabetes education programmes for multidisciplinary teams in the hospital setting

To participate in relevant local, regional and national meetings for further development of expertise and sharing best practice

To contribute to publications relating to nursing practice and the delivery of the diabetes nursing service

Research & Audit:

To collect clinical audit date measuring the outcomes of patient care ensuring this information is accurately entered

To maintain an awareness of current clinical trials within the diabetes centre and to be aware of research trends and to undertake research where gaps in service are identified

To evaluate and implement agreed standards of care that have resulted from audit data To be actively involved in research trials

To prepare and actively participate in Trusts annual Performance Development Review process and progress reviews.
