We are looking for forward thinking, passionate Nursing Associates to support the management of patients within the Haverhill Locality. Main duties of the job
The role requires you to be able to keep accurate clinical records and have good clerical/ administrative skills and therefore experience of clinical recording systems, ideally SystmOne will be beneficial.Your day will include:Gathering and collating information, planning and delivering healthcare for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible.Working within the integrated team to facilitate early discharge from hospital.Working within the integrated team to prevent unnecessary admission to hospital.Working with all health care professionals, and statutory/non-statutory agencies to provide a seamless, integrated service to our service users.
