Job summary
Here at WSH the role of Care Coordinator is an integral part of every ward! They are skilled at communication, multi-tasking, and often have a keen eye for detail. They not only work with the consultants, but are often the primary contact for family members. Our Care Coordinators play a pivotal role in the discharge planning of some of our most complex patients, ensuring that they have been seen by therapists, social workers and our complex discharge planning team. They also manage all ward-led discharges, liaising with care/nursing homes and working with our Transport department to make sure that our patients get home safely.
The post holder will also communicate the immediate position on the bed occupancy on the ward including up-to-date discharge information. This helps with patient flow throughout the trust. Furthermore, fulfilling this administration function enables medical and nursing time and attention to be spent directly caring for our patients.
The post holder will be working as part of the medical division, with different teams of staff, all working to deliver the best quality and safest care to their patients.

Main duties of the job
- To work as part of the ward team maintaining responsibility for good communications between all departments, driving forward safe and timely discharge by ensuring all administrative work is completed
- To attend Multi-Disciplinary Team (MDT) meetings, and actively participate in daily board rounds, and record actions taken and appropriately follow-up.
- To update plan of the day, discharge readiness and Reason to Reside (R2R) on the whiteboard to ensure MDT staff/patient flow team/operational managers are aware of the patient's progress at all times
- To liaise with the medical teams and pharmacy staff to ensure discharge medication is prescribed and dispensed
- To liaise with Social Services Departments and ward-based staff with regards patient's needs, families and carers, gaining consent as appropriate to make referrals to Social Services in accordance with the Community Care Act 2014 as appropriate, to ensure care packages are in place in a timely fashion to expedite discharge
- To escalate issues to the nurse in charge or the supporting Manager of the day which prevent the patients' journey from progressing

The post-holder will be managed by the Assistant Service Manager, in liaison with the Ward Manager to ensure appropriate resource throughout the Trust, and to ensure the patients journey through the hospital is as smooth as possible without any unnecessary delays.
