We are looking for forward thinking, passionate Nursing Associates to
support the management of patients within the Bury Town Locality. You will
provide nursing treatments under the delegation of a district nurse and work to
the full scope of your registration.

As nurse
associate in the Integrated Neighbourhood team you will be responsible for
providing safe, effective nursing activities in peoples own homes, care homes
and community clinics. You will work closely with all the members of the INT to
achieve the best long-term outcomes for all your patients.

Main duties of the job
The role
requires you to be able to keep accurate clinical records and have good
clerical/ administrative skills and therefore experience of clinical recording
systems, ideally SystmOne will be beneficial.
Your
day will include-
Gathering
and collating information, planning and delivering healthcare for patients, for
example with long term conditions, palliative care and rehabilitation needs, to
achieve quality of life and independence where possible.
Working
within the integrated team to facilitate early discharge from hospital.
Working
within the integrated team to prevent unnecessary admission to hospital.
Working
with all health care professionals, and statutory/non-statutory agencies to
provide a seamless, integrated service to our service users.
This post

Job summary:
As nurse associate in the Integrated Neighbourhood team you will be
responsible for providing safe, effective nursing activities in peoples own

Job responsibilities:
Gathers and collates
information that contributes to the assessment of patient care needs.
Undertake and
interpret a range of patient observations, recognising the deteriorating
patient and swiftly referring to the GP as appropriate.
Follows the WSFT
policy to perform administration of medicines within a defined scope of
practice and within defined exclusion criteria.
Take responsibility
for the delivery of care to a group of identified patients, supporting the
affective management of the patients journey through admission, treatment and
discharge.
Maintains clear and
concise patient records and handover of patient care
Work as part of a
multi-disciplinary team.
Communicate
effectively with patients, relatives, other carers and staff as appropriate.
Enable people to
participate effectively in the promotion of their health and wellbeing and the
prevention of adverse effects.
Respect peoples

Contribute to the ongoing assessment of
patients (under the supervision of a senior registered practitioner),
identifying nursing problems and needs in partnership with the patient, and
with appropriate members of the inter-professional team.
Undertake the completion of delegated
patient risk assessments (under the supervision of a senior registered
practitioner) to support their health and well-being within scope of nursing
practice.
In collaboration with the District Nurse
in charge of the patient, contribute to the planning, implementation and
evaluation of individual programmes of care for patients.
Ensures that all drugs and treatments
are administered as prescribed, within their defined scope of practice.
Ensures the maintenance of accurate
clinical observations of patients condition.
Ensures the significance of data
relating to patients condition and progress is interpreted and escalated
appropriately.
In collaboration with the registered
nurse, evaluates the effectiveness of interventions and updates care plans and goals
to take account of any changing needs of the patient.
Undertakes a range of procedures
relevant to the clinical area of work, following, training, supervised practice
and formal assessment of competency.
Ensures safe practice to minimise the
risk of infection to patients and staff.
Provides and promotes health education
specific to the clinical area and in line with local and national
