Working with a team of
committed colleagues you will be:
Tracking patient care along
agreed suspected 2 Week wait, 28 Faster Cancer diagnosis and or diagnosed
cancer 31 and 62 days first and subsequent treatment pathways collating pathway
information for Cancer Waiting Times,
Outcomes and Services and other agreed Cancer Datasets and enter
them in designated system and ensure the
completeness and accuracy of data items entered to monitor patient pathway and
agreed returns in a timely manner.
Working alongside the
designated cancer site specific teams, Trust core services such as radiology,
pathology, endoscopy and oncology, patient administration teams ensure patients
are tracked and expedited through agreed pathway timelines.
Ensuring that agreed inter
provider transfer dataset for specialist MDT review and or treatment is complete
and effectively communicated to all parties, and remain the key contact point
for the cancer (tertiary) or referral centres in relation to referred patients.
Main duties of the job
A key member of the team they are expected to be familiar with cancer data collection and reporting requirements and work flexibly with assigned cancer MDT Lead and other clinicians, Cancer Nurse Specialists and collect Cancer waiting times, COSD, other agreed data items to monitoring pathway waiting times and targets.
Being responsible for designated site-specific patient tracking keep up to date with the national guidance on site specific pathways for cancer waits, clinical audit, Chemotherapy and Cancer Outcomes and Services data collection advising the MDT of the implications of such guidance including the need to input the relevant data items on to the Somerset Cancer Register database.
Ensure data accuracy, completeness and other quality requisites throughout the system ensuring other users of the patient record maintains an accurate and timely input of datafully familiarise with all cancer data collection and reporting requirements.
As champion of cancer pathway working closely with relevant teams continuously improve on accuracy and completeness of cancer data collated and entered in the system.
Ensure PTL provides consistent pathway information and escalate pathway issues at the earliest opportunities with the responsible clinician and their Service Managers enabling proactive management of potential delays with a view to ensuring that the targets are met.

COMMUNICATION AND TEAM
WORKING
Work
with designated cancer MDT Lead and other clinicians and Cancer Nurse Specialists
and collecting Cancer waiting times, COSD, other agreed data items and support

PLANNING AND ORGANISING
Keep
up to date with the national guidance on site specific pathways for cancer
waits, clinical audit, Chemotherapy and Cancer Outcomes and Services data
collection advising the MDT of the implications of such guidance including the
need for review and update of relevant policies, protocols and input the
relevant data items on to the Somerset Cancer Register database.
Ensure

ensuring other users of the patient records maintains an accurate and timely
input of data.
Be
fully familiarise with all cancer data collection and reporting requirements.
Be

accurate and timely capture of data for patients being transferred by other
Co-ordinators and support staff maximising the opportunities for completeness

closely with the lead Manager and Supervisor in ensuring trust data register system
is fully operational and serves the purpose at all times.
Attend
National, Network and local events as appropriate to the requirements of the
post. Promote links with other Trusts in
the network alliance and participate in the development of processes for
accurate transfer of data for inter-trust referrals.
Maintain physical resources provided to achieve the job purpose.
Report the issues
around the quality of data collected by others ensuring timely and accurate
data collection and that any learning/development needs are identified and
dealt with by the services, as appropriate.

effective Videoconference links to the site-specific MDT functionalities.

appropriately.
