A full time Band 3 Generic Worker / Support worker, position is now available within the Haverhill Health Community Team.

We are looking for an applicant with experience in a variety of NHS clinical areas though previous community experience, though ideal, is not essential.

The successful applicant will work with the District Nursing team, our Community Matron, OT's & Physio's, alongside the GP practices to provide high quality patient care, together with our Integrated Alliance partners, ACS, Home Care, Council & Voluntary sector colleagues.

The Haverhill Integrated Team is a friendly, harmonious team, who always look to support each other in caring for individual patients our neighbourhood, which has a warm community feel. The base site at Haverhill will soon be relocated to the Haverhill Health Centre; enabling an integrated working environment.

Main duties of the job

We receive referrals for adults with all long-term conditions; leg ulcer patients; diabetics; patients discharged from acute hospitals with ongoing needs and in the prevention and management of falls etc. Treatment is carried out in patients own homes.

You will be responsible for your own work with supervision & support from the Community Nurses & Therapists.

You are expected to have excellent communication & IT skills.

You will show experience of basic nursing skills & an ability to contribute to care planning and carrying out interventions to aid patients' health and wellbeing. You will have access to training and supervision that will build upon your current skills.

You will need to have the ability to travel to all rural locations to carry out the post. Pool vans are available for staff to use on a shared basis & to transport equipment.
