Job summary
As a part of the
busy inpatient diabetes specialist nurse team, and supported by senior diabetes
specialist nurses, you will collaborate with clinical colleagues to offer
specialist knowledge on the management of diabetic patients.
You will provide
education for patients and their carers, empowering individual in their diabetes
management.
Supporting nursing
colleagues across the trust, in providing education and information for
patients and/or their carers in the management of diabetes.
Main duties of the job
To work within the existing Diabetes
inpatient team, improving
the care for people
with diabetes who are admitted to hospital.
Facilitating safe discharge is a key component of the role.
Reviewing patients
at the earliest opportunity when they arrive
in Emergency Dept or the Emergency
Admissions Unit.

Clinical
