Job summary

Applications are invited for a new consultant post at this Trust. This is an exciting opportunity to join an excellent Emergency Department team in a Trust that genuinely sees Emergency Medicine as a major priority. We were rated as "Outstanding" for Emergency Care Leadership by the CQC (and "Good" in all other aspects of Emergency Care). The Department has recently undergone significant estates work and has a new 14-cubicle majors area and a new expanded Paediatric ED, with work currently underway to provide an updated and refurbished minors area. We have an award-winning Acute Medicine and Ambulatory Care unit situated right next to the ED, with very good relations between the Emergency and Acute Medicine consultants.

Main duties of the job

  1. Provision within the organisation of the department of a comprehensive Emergency Medicine service to the Trust, with responsibility for the prevention, diagnosis and treatment of illness and the proper functioning of the department
  2. Liaison with colleagues in other specialties and with management to achieve such a servic
  3. Administrative duties associated with the care of patients and the running of the Emergency Medicine service
  4. The provision of advice, via professional and management structures at Divisional and Trust level, on the short and long term planning of comprehensive health provision for the local population
  5. Participation, as required, in the instruction of all professional groups, including the department's own postgraduate programme
  6. The provision of cover, on a reciprocal basis, in the absence of departmental colleagues in order to ensure the maintenance of an Emergency Medicine service and continuity of the department's commitment to the Trust's Major Incident Plan
  7. Maintenance, via a process of continuing professional development of the knowledge and skills relevant to the post
  8. Acceptance of and compliance with, the department's and Trust's programmes for Clinical Governance, CPD and Medical Audit
  9. Out-of-hours responsibilities, including participation in Consultant on-call rota where applicable.

About us

The Hospital was rated "Good" by the CQC in February 2022 and the Emergency Department was rated "Outstanding" for leadership and "Good" in all other areas.

The position provides an excellent opportunity to undertake an educational role for the medical and nursing staff. The Emergency Department receives medical students from the UEA Medical School. Consultants participate in the lecture programme delivered at the University.

The Department sees around 75,000 patients per year. It receives major trauma and is a designated Trauma Unit with a dedicated helipad in the grounds. It serves a population of 230,000 people across Great Yarmouth, Lowestoft and the surrounding rural communities.

  • Many changes to working practices have taken place: the unit utilises Clinical Support Practitioners, Clinical Support Assistants and Emergency Advanced Nurse Practitioners and now has on-site Mental Health Assessors for 24 hours per day. The department has taken an active role in the UEA Physician Associate training scheme. The first graduates started working with us in Feb 2018. We have an established ACP training programme.

Subject to the provisions of the Terms and Conditions of Service to observe the Trusts agreed policies and procedures drawn up in consultation with the profession on clinical matters and to follow the standing orders and financial instructions of the Trust.

To observe local and national employment and personnel policies and procedures, and to comply with Regional and Trust Health and Safety policies.

To assume responsibility for and facilitate contact from members of staff with delegated duties for personally accountable patient care.

Job Plan

A formal job plan will be agreed between the appointee and their Divisional Director, on behalf of the Medical Director, three months after the commencement date of the appointee. This will be signed by the Chief Executive and will be effective from the date signed by the Divisional Director. For the first three months the job plan will be based on the provisional timetable.

The job plan will then be reviewed annually, following an Appraisal meeting. The document will be a prospective agreement that sets out a consultants duties, responsibilities and objectives for the coming year. It will cover all aspects of a consultants professional practice including clinical work, teaching, research, education and managerial responsibilities. It will provide a clear schedule of commitments, both internal and external. In addition, it will include personal objectives, including details of their link to wider service objectives and details of the support required by the consultant to fulfil the job plan and the objectives.

Includes clinical activity, clinically related activity, predictable and unpredictable emergency work.

Supporting Professional Activities 2.5 PAs on average pw

Includes Medical school teaching and familiarisation with the Trust policies, guidelines and procedure and also includes CPD, audit, other teaching activity and research.

This is for a 10 PA contract. The allocation of PAs is reviewed and may be subject to adjustment when a further diary exercise is undertaken or if the service demands a review of the Job Plan. We are very open to applications from colleagues seeking either fewer or more PAs, up toa maximum of 12 PA. The teaching component may vary. Research interests would be accommodated if possible within the job plan.

Timetable

The timetable is only indicative and is based on a 10 PA contract under our existing 1 in 10 rota. There is 0.8 DCC PA allowance for unpredictable on-call work (in addition to the standard consultant on-call supplement). 0.5 SPAs can be carried out at any time of the consultants choosing. As this is a new post, we will need to redesign the rota (with input from the new appointees) to reflect the new number of consultants. The definitive timetable will be agreed by the Trust and the appointee and subject to regular review. We aim to be as flexible as possible in developing an individualised job plan that works for you and are happy to accommodate fixed days off if desired.

The department currently operates a 1:10 on call. At present the majority of consultants have a set late shift each week and do a weekday overnight on-call every other week. The successful candidate will be expected to participate in this. Currently weekend working is on a 1:8 basis, with the consultant working from 8.00h to 16.00h on the shop floor both Saturday and Sunday, and usually on-call thereafter. As our numbers increase we will look towards having 2 consultants on each weekend (0800-1600 and 1600-2200), and the frequency of the overnight on-call at weekend will reduce in line with the number of consultants.

Our Departmental policy, in keeping with RCEM guidance, is that the on-call ED Consultant is available for Major Trauma, telephone advice, and will attend the department at the request of ED Specialty trainee or Specialty Doctor or sister if Consultant presence is genuinely required from a patient safety perspective. We have a locally agreed system for additional reimbursement if the on-call ED Consultant comes in purely to assist with avoiding breaches of the 4hr performance target.
