Job summary

There are two Consultant posts available. One is a replacement post with a malignant interest and the second post is a new non malignant post, with an interest in anticoagulation/ thrombophilia / bleeding disorders. However depending on the candidates appointed, the job plan will be tailored to suit the candidates and needs of the department. Consultant staff are expected to continue to provide level 2 services at this hospital site and also to enable the department to participate in undergraduate teaching at the UEA Joint Venture Medical School. The posts will provide an excellent opportunity to actively contribute in the further development of a modern Haematology service within this highly regarded Trust with its wide range of acute and community services, which will afford high professional visibility.

With the formation of a cancer alliance the staff at the James Paget University Hospital work in close association with those of the Norfolk & Norwich University Hospital. There are common strategies and protocols for the care of patients with malignant haematological disorders discussed at multi-disciplinary meetings. Close co-operation between the two units enables a number of initiatives aimed at staff training and education.

Main duties of the job

There is a dedicated Haematology inpatient ward (ward 17) with direct admission rights, an acute oncology service and a busy haematology and chemotherapy day care unit (the Sandra Chapman Centre). The post holders will be expected to be responsible for the care of haematology inpatients, day care patients and outpatients, including participation in clinical trials.

About us

The department provides level 2b BCSH haematology services. The unit is very busy and contributes well to NCRN trials and has the support of the Clinical Trials Office and a nurse to assist in data collection and reporting. There is a MDT meeting twice a week with the Norfolk and Norwich Hospital on Tuesdays. Once a month, there is a teleconference MDT with Addenbrooke's Hospital which is our allogeneic transplant centre, and a similar meeting with the NNUH where our autologous bone marrows are performed.

Whenever possible patients are treated as day cases and the day treatment centre (the Sandra Chapman Centre) is adjacent to the inpatient ward. The Day Centre has recently been extended and consists of single rooms and treatment chairs. The Day Centre is responsible for review of patients with a variety of malignant and non-malignant haematological patients including acute leukaemia and patients with lymphoproliferative disorders. This is an extremely busy unit with over 4000 consultations a year.

Inpatients are admitted to a ward of 17 single rooms, which is shared with oncology. There is no upper limit to the number of inpatients and haematology and oncology patients have priority for these beds. On average we have ten inpatients. Admission of Haemato-oncology patients is in line with the National Acute Oncology Service (AOS) guidelines. First on-call for inpatients are dedicated middle grade haematology/oncology trainees who do a 1:4 resident cover 8 am-8pm.

Job description

Job responsibilities

  1. Share with consultant colleagues the Haematology workload.
  2. Consultant input to the Anticoagulant, Antenatal Haematology and Transfusion services.
  3. Work as part of a multidisciplinary team with junior medical staff, nursing staff, pharmacist, physiotherapists, occupational therapists, social workers and palliative care nurses.
  4. Attendance and discussion of all appropriate patients at MDT meetings for Haematological Oncology and any other MDTs that may be set up.
  5. Provide Consultant support for the pathology laboratory which functions as part of the Eastern Pathology Alliance (EPA) including attendance and participation in local and EPA level forums.
  6. Provide a consultation and advisory service to medical colleagues in other specialities in the Trust and to general practitioners.
  7. Participation in clinical meetings, postgraduate and undergraduate activities of the Trust, and in particular offer teaching to UEA Medical Students in various modules.
  8. Participation in the teaching and audit activities of the Department, including quality control in the laboratory.
  9. Participation in clinical trials.
  10. Provide with the other Consultants a system of mutual out-of-hours cover for annual, study and professional leave. Short periods (up to 2 weeks) of sick leave will also be expected to be provided by the remaining consultants
  11. Share a 1:5 on-call rota Band A with prospective cover. Junior and middle grade support from within haematology are first on call during the on call period.
  12. Participate in the Trusts Clinical Governance processes and in CPD. The Trust will provide study leave expenses in line with national terms and conditions of service.
  13. Contribute to management of the service and, by agreement, take on the role of clinical lead in due course.
  14. Comply with the outline Consultant work programme (under provisions of Circular HC90/16) and the weekly timetable of fixed commitments (i.e.: regular scheduled NHS activities in accordance with paragraph 30B of the TCS).

To observe local and national employment and personnel policies and procedures, and to comply with Regional and Trust Health and Safety policies.

To assume responsibility for and facilitate contact from members of staff with delegated duties for personally accountable patient care.

Job Plan

This replacement Consultant will ideally have a special interest; however the Job plans and detailed timetable will depend on both experience and interests of the candidate and the needs of the department. This will be agreed with the Divisional Director on behalf of the Medical Director, within three months of the commencement date of the appointee. This will be signed by the Chief Executive and will be effective from the date signed by the Divisional Director.

The successful candidate will be offered a 10 PA contract in the first instance in accordance with the terms of the new consultant contract. Additional PAs may be available to interested applicants. The mix of clinical sessions in the final job plan and rota will be agreed after discussion with the Divisional Director, and the weekly timetable will include the following mix of programmed activities.

Supporting Professional Activities

CPD/Teaching/Research/Audit/Clinical Governance: 1.5SPAs on average per week

The Job Plan will be reviewed annually, following the Annual Appraisal Meeting. The Job Plan will be a prospective agreement that sets out a consultants duties, responsibilities and objectives for the coming year. It will cover all aspects of the Consultants professional practice including clinical work, teaching, research, education and managerial responsibilities. It will provide a clear schedule of commitments, both internal and external. In addition, it will include personal objectives, including details of their link to wider service objectives, and details of the support required by the consultant to fulfil the job plan and the objectives.

On Call Availability

On-call responsibility will be 1 in 5 with prospective cover for leave with the Consultant. The on-call period includes all hours outside Monday to Friday from 09.00 -1700hrs.

Anticoagulant and Thromboprophylaxis Service

All anticoagulant dosing locally is currently done via the hospital anticoagulant service based on laboratory INRs, although the majority of patients are bled in the community. Dosing is via computer dosing system (APEX). Manual dosing advice is given by the anticoagulant nurse specialists, when required. Inpatients on oral anticoagulants are also dosed by the anticoagulant nurse specialists. A fast track DVT service is run by the anticoagulant nurse specialists.

There are four anticoagulant nurse specialists.

There is active Thromboprophylaxis committee and regular audit of thromboprophylaxis. One of the Consultant posts will include oversight of these services.

Laboratory Haematology and Blood Transfusion

The Haematology and Blood Transfusion Laboratories at James Paget Hospital is managed by the Eastern Pathology Alliance (EPA), an alliance of three hospitals including Norfolk and Norwich University Hospital and the Queen Elizabeth Hospital, Kings Lynn.

Histopathology and Cytology services are based at the Cotman centre in Norwich. Haematology and Blood Transfusion is currently CPA accredited and MHRA compliant.

The Laboratory also operates a comprehensive anticoagulant dosing service to hospital and GP patients.

Haemato-pathology Services

Haemato-Oncology services are currently all provided at Addenbrookes Hospital in line with IOG directives which mandate the delivery of such services through an SIHMDS.

The department provides a 24 hour advice service for the hospital and local GPs including out of hours.

The successful candidate will have an office with their own desk, personal computer and secretarial support.
