Job summary

The Cancer Services CBU are looking for a highly motivated and enthusiastic individual to join them as a Patient Tracker to ensure all patients are seen, and treated in a timely manner.

This role will be pivotal to the smooth running of the patient pathway across our two sites (PCH and HH) and will directly enhance our patient experience. Strong organisation skills are key, and the ability to build rapport across different staff groups.

Main duties of the job

The post holder will be responsible for the proactive management of patients receiving Systematic Anti-Cancer Therapy (SACT) treatment in the Haematology/Oncology Units across North West Anglia NHS Foundation Trust.

Monitoring, reporting, and tracking patients, ensuring the patients journey is progressed in a timely and efficient manner in line with National Cancer Waiting Times Targets and in the best interests of the patient.

Job description

Job responsibilities

To navigate patients through their care pathway, from point of referral to treatment, in accordance with the pathway steps.

To follow outcomes from MDT meetings that require an appointment within the department but are awaiting a referral from the referring speciality. Ensuring appropriate capacity if available and the referral has been triaged by a Consultant for clinical urgency.

Reviewing capacity across both sites (PCH/HH) and ensuring the patient is offered the first available outpatient appointment with the appropriate specialist.

Reviewing capacity across both sites (PCH/HH) Working in collaboration with the Outpatient and Chemotherapy Booker to ensure the patient is offered the first available start date for SACT treatment.

To proactively identify and resolve delays at all stages in the patient pathway. Where this is not possible, to follow the escalation procedure highlighting any relevant issues to line management.

To be responsible for resolving any escalations raised during the PTL meetings by the Cancer Services Performance Manager, providing updates accordingly.

To highlight any bottlenecks within the process, communicating regularly with line management, and where possible suggesting and implementing solutions.

To review all planned activity in the Day Units to ensure the appropriate information is available to Pharmacy in an appropriate timeframe to minimise treatment delays. Liaising with Consultants if prescriptions need to be reviewed prior to processing.

To assist with the practical arrangements of transferring a patients care to an alternative Day Unit across site due to urgency or any other reason. Including the transfer of consent and prescription information to the relevant prescribing system.

To liaise with the relevant clinical teams to ensure quality of data collected.

As an ongoing process consider opportunities for improvements in the co-ordination of the pathway and in the efficient transfer of patients and information between departments.

To maintain accurate clinical records on Trust IT systems to provide robust auditable pathways.

To support quality improvement processes within the CBU

To provide cover for absent colleagues to ensure a smooth running of the service.

To undertake related training as required.

To develop a good working knowledge of anatomy, terminology, treatment types, medical coding and pathology relating to cancer. (To attend relevant courses as required)

To undertake additional information requests and associated duties as and when required.
