Job summary

The Integrated Neighbourhood Project Manager will provide transformation support and project management for the Integrated Neighbourhoods programme of work. This is a key priority for the Integrated Care System and is being delivered via the North and South Care Partnerships.

The post holder will support the development and delivery of the Integrated Neighbourhood, which will have a Primary Care Network (PCN) at its core. This will include working closely with stakeholders with engagement events and bringing all partners who work with the identified community together.

The Integrated Neighbourhood Project Manager will work closely with the GP Clinical Lead and clinical teams to review their population health data and will support public engagement to determine how the local population want their health and care services to improve. The role will be responsible for supporting the service redesign and delivery of specific projects within the Integrated Neighbourhood, coordinating activities to facilitate the successful delivery of the personalised objectives.

In addition to supporting a specific Integrated Neighbourhood within a locality the post will support the delivery of the integrated working improvement projects which have been developed by each Place. This programme of work aims to make operational changes to improve cross-organisation working and remove the barriers to integration.

Main duties of the job

Key Responsibilities

  • Positive relationship building within the Integrated Neighbourhood(s).
  • Project lead for specified Integrated Neighbourhood(s) within one of the Locality areas in the North Place.
  • Project management support to the Integrated Neighbourhood to enable delivery of specific priorities.
  • Project support to cross-organisational meetings including co-production and presentation of highly complex and sometimes contentious information to support decision making, and design of agendas to enable key decision making.
  • Support, facilitation and delivery of the Integrated Neighbourhood.
  • Support, facilitation and delivery of integrated working improvement projects.
  • Assurance that specified programmes of work are being delivered within budget and in accordance with the overall plan.
