Job summary
The purpose of this post is to support the Head of Quality Governance & Compliance in managing the Trusts mortality review process including monitoring outcomes from case note record reviews (mortality reviews), demonstrating compliance with the national Learning from Deaths framework and monitoring hospital mortality ratios.
The job involves supporting the Head of Quality Governance & Compliance to provide data and reports to various Trust level and Clinical Divisional committees.
The post holder will provide administrative support including creating and maintaining accurate databases, extracting data for local divisional and high level reports and taking minutes at relevant meetings and committees as required.
This role will involve the post holder acting as the first point of contact across to the Trust in relation to mortality and providing advice and guidance to resolve issues or queries relating to the Trust's mortality review process.
Main duties of the job
Including but not limited to:
To be the first point of contact for queries around mortality/learning from deaths
To plan for SJR sessions including scheduling sessions, seeking reviewers, preparing pre-session documentation
To accurately record the outcomes of SJRs and extract learning points, acting on your own initiative in identifying flags where improvements need to be made, providing data to relevant committees
To organise training sessions for SJR reviewers, including training documentation, attendance records and updating presentations, and maintain up to date training records including the production and issue of certificates
To provide timely data as required on deaths under the Learning from Deaths framework, including reviews undertaken and their outcomes to external organisations such as LeDeR.
To log new investigations, download data from Dr Foster, review for themes and trends and in conjunction with the Head of Quality Governance & Compliance, share with the relevant clinical coding and clinical teams in the agreed timeframe.
To arrange deep dive and ad-hoc reviews as required at the request of the Head of Quality Governance & Compliance.
Undertake reviews of the Standardised Hospital Mortality Indicator (SHMI) preview data, liaise with Information services to ensure the accuracy of the data and review status to determine if each diagnosis group requires coding or clinical review and record this on the spreadsheet and initiate the necessary reviews
