This exciting post provides an opportunity to work within a multidisciplinary team of qualified and unqualified staff. You will assist the Safeguarding and Social Work team in collating and compiling safeguarding information and facilitate safe and timely discharge of patients from the hospital. You will have excellent proficiency in Microsoft programmes such as Word, excel, and PowerPoint to support the Safeguarding and Social Work team when preparing reports and documentation.
It is a also a patient facing role which involves gathering and collating information from medical notes, patients and multidisciplinary team colleagues, to enable a clear, safe and timely plan for discharge to be followed and establish any additional support needed on discharge.
This role is part-time (30hours per week);
Main duties of the job
The role of the social care coordinator involves providing a high quality, efficient and complete administrative support service including diary management to the Safeguarding and Social Work Team working within a multidisciplinary team of qualified and unqualified staff.
The post holder will work to improve the discharge experience for patients and their families, by facilitating a safe, timely transition to the patient's home or ongoing care facility including managing information and referrals both internally and with external stakeholders.
The post holder will attend morning board rounds and will interact with colleagues such as occupational therapists and physiotherapists, patients and their relatives in a professional manner.
